Most structural failures don’t trace back to one bad calculation. They trace back to a verbal design change that never reached the drawings, a material substitution accepted on-site without re-testing, or a test report filed but never checked against what the design actually needed. Each stage of a project—design, materials, execution, testing—usually has its own checks. What most projects lack is control over the handoffs between them, which is exactly where structural risk accumulates.

For a project head, this is the real exposure. You’re rarely the person who makes the technical error, but you’re almost always the one accountable for it. A design approved by consultants, materials tested by labs, execution run by contractors—the responsibility for tracking what happens between these functions sits with you.
